A publisher can underestimate demand and run short when REorders come in (as happened with Bloodline), but with rare, RARE exceptions, advance orders are always filled.
I worked for an Indie for a while a few years back - dustjacketing and cleaning up firsts and rare books, and assisting with their community literacy program. the guy had made some money early in silicon valley biz and had a great vision- its a shame he died before he could see it through. so the info he was giving me was just plain wrong and he knew that I knew it.
