Paul is his middle name and is what most of us call him. If you want to use an honorific, it'd be Dr. Wilson (he's a real medical type doctor, not just one of those people who wrote a thesis about underwater basket-weaving or something similarly trivial and got Piled Higher and Deeper).

In Tennessee the honorific is MR or Miss If their your elder. Doesn'T matter if its the Queen of England or Dr F Paul Wilson. Its something one is raised to do.
PS> I'm not an idiot, I know Mr Paul is a doctor of medicine, not a basket weaver. My concern is your concern with having to let me know like a little child. If I need to be schooled, I am more than capable of asking. Thanks anyway.
